Definition

Charge capture is the process of recording every product used in patient care so that it can be billed accurately to the patient, payer or both. Missed or inaccurate charges are a direct hit to margin, particularly in procedural areas where high-value items are used.

Why it matters

Charge capture failures are one of the most consistent sources of revenue leakage in US health systems. The traditional pattern is post-procedure data entry, often days after the event, with errors and omissions that are difficult to reconcile.
